Pneumocystis carinii pneumoniae (=PCP, Pneumocystis jiroveci)

definitie: longontsteking met PCP bij patienten met een verlaagd immuunsysteem

Oorzaken:
Infectie met PCP. Bij AIDS, orgaan transplantatie, chemotherapie, glucocorticoid therapie.

Verschijnselen:
-Koorts
-Dyspnoe (kortademigheid)
-Wel of niet droge hoest

Complicaties:

Diagnostiek:
X-thorax: vlekkerig interstitieel infiltraat. Soms lobulair, soms caveneus.
Fluorescentie onderzoek van het sputum (slijm), BAL vocht of boipsie.

Behandeling:
-Bij niet-HIV patienten
Trimethoprim (15-20 mg/kg) + sulfamethoxazole (75-100 mg/kg) IV of PO dagelijks in 4 doses. Duur: 14 dagen

-Bij HIV patienten
Zuurstofsaturatie meten. Corticosteroiden toedienen indien te laag G6PD testen. Indien aanwezig darson of primaquin toedienen Trimethoprim (15-20 mg/kg) + sulfamethoxazole (75-100 mg/kg) IV of PO dagelijks in 4 doses. Duur: 21 dagen
